These terms and conditions are a legally binding agreement ("Agreement") between you and LifeBridge Health. By accessing or using the LifeBridge Health App, you agree to be bound by all of the terms and conditions of this Agreement. If you do not accept these terms and conditions, you must not access or use the App. LifeBridge Health may modify this Agreement at any time, and such modifications shall be effective immediately upon posting the modified Agreement. LifeBridge Health grants you a limited, revocable, non-exclusive, non-sublicensable, and non-transferable license to use the App, subject to and in accordance with this Agreement. No licenses or rights are granted to you by implication or otherwise, except for the licenses and rights expressly granted to you herein. LifeBridge Health reserves all rights not expressly granted by it to you. In certain jurisdictions, you may have legal rights as a consumer, and this Agreement is not intended to limit such consumer legal rights that may not be waived by contract.
All health and health-related information contained within the LifeBridge Health App is intended to be general in nature and should not be used as a substitute for a visit with a health care professional. The advice is intended to offer only a general basis for individuals to discuss their medical condition with their health care provider. Your health care provider should be consulted regarding matters concerning yours and your family's medical condition, treatment, and needs. LifeBridge Health does not review, control, or take responsibility for the accuracy of information provided by these websites, or the accessibility, privacy or security of these websites. Although every effort is made to ensure that the LifeBridge Health App is accurate, secure, and timely, it is provided for the convenience of the App user and LifeBridge Health makes no guarantees as to its correctness, completeness, security or accuracy. Also, unauthorized additions, deletions, or alterations could be made to the App by third parties without our knowledge. If you believe that information found on the App is inaccurate or unauthorized, please inform LifeBridge Health by contacting us as provided below. LifeBridge Health is always trying to improve our services. Potential users in any jurisdiction in the world, the common, statutory, regulatory, or codified law of which would void this agreement in whole or in any essential part or which makes accessing this App illegal, are not authorized to use this site, and do so entirely at their own risk. To the extent applicable, the foregoing applies to any information LifeBridge Health may disseminate through any social media (e.g. Facebook, Instagram, Twitter, etc.).
This statement discloses LifeBridge Health's information-gathering and dissemination practices for this App. You may visit the App without submitting any information about yourself. If you send LifeBridge Health email you will be asked to submit information about yourself. LifeBridge Health will use this information for replying to your message or forwarding the requested material.
